Three Consultation Optimal Health Package

Includes a 75 minute initial intake appointment, two 60 minute follow up appointments, wellness plan, and email correspondence between consultations. This package may also include supplement recommendations, lifestyle recommendations, shopping tips, recipes, and/or meal plans.

If you wish to continue working together after our first three appointment, we can discuss a plan that’s
right for you.

Initial Intake Appointment – 75 minutes

Our first consultation will cover a broad overview of your health and wellness, goals you’d like to accomplish, and things you’ve tried in the past. Before our first appointment, it is helpful for me to review any supplements/medications you are taking and recent lab work. I may also want to you to complete a 3 day food/activity journal. Please schedule this consultation at least 7 days in advance, so you have time to complete the paperwork. It is important to complete and send me that information 3 days before our first appointment. This is a helpful tool for both of us to clearly see where you are before our first meeting. By the end of our consultation, we will determine the goals you want to accomplish and talk about how to make them happen.

Follow Up Appointments – 60 minutes

These appointments are for us to check in, honor your accomplishments, and talk about challenges that came up after our initial consultation. We can meet weekly, bi-weekly, or every 3rd week, depending on what works best for you. The follow-up appointments will allow us to continue to expand on your goals and our work together to support them. Each consult will end with actionable items for each of us to complete that we both agree will be helpful.
